A survey given to parents for collecting information about preventive and developmental care for their children under the age of four. It also captures information about the child's health (e.g. whether the child has a special health care need, child's risk for developmental, behavioral or social delays), child's health care, parent health and family behaviors. The PHDS was developed by the CAHMI, while at FACCT.
