Abstract:
The signature program in the Robert Wood Johnson Foundation’s commitment to improve the quality of U.S. health care,Aligning Forces for Quality (AF4Q) brings together patients, health care providers and payers to turn proven practices for improving quality into real results. It seeks to lift the overall quality of health care in entire communities, reduce racial disparities and provide models for national reform.
These snapshots describe how each of the AF4Q communities is improving care for all patients by advancing performance measurement and public reporting, consumer engagement, and quality improvement.
